Could work in our favor. If he misses entire year, can't he petition for a medical waiver which gives him an extra year? Thus we would have him for 2 years after Flowers instead of 1?

I don't think so.  An extra year is granted if you have two seasons that you missed because of injury.  A RS season (which last year was, essentially) and an injury season doesn't get you another year.

You can still get a Medical RS if you had a regular  RS season.

Case Keenum is a perfect example.  RS his true freshman year.  Tore his ACL in his RS Senior year and got a medical RS to play one more year.
